Download the Credit Application and Fax it in.


	Download the Customer Profile Sheet Download the file in the format of your choice. Print it. Fill it out the application completely. Fax it back to the number printed in the form, along with: Your Reseller Certificate, your State Tax Certificate, or any document hat accredits you as a VAR, OEM, Integrator, Systems Reseller, etc. If and ONLY if you are planning to have your Credit Card charged for all of your purchases, download the Credit Card Release form, and fax it back to us including copies (Front and Back) of your Credit Card, (Visa, or Mastercard ONLY; We do not accept American Express) and a valid Driver License issued by the State you reside in. NOTE: The Credit Card Billing Address must be the same as it appears on your statement for the transaction to be approved. The use of Credit Cards against any purchase carries a 3% (three percent) surcharge.
